Despite Watchman Slowdown, BSX Still Strong Value Play

We believe the market is discounting the stock unfairly

Thesis Statement

Boston Scientific maintains its wide moats and remains one of the top players when it comes to intellectual property in medtech. BSX faces unanticipated headwinds in their WATCHMAN segment, which grew below expectations due to hospital capacity and reimbursement dynamics, leading to a downward revision of 2026 expectations. While the recovery from headwinds will take time, the stock trades well below its intrinsic value, and is meaningfully discounted, assuming these cyclical headwinds do not evolve into structural impairment.

Market Dislocation

BSX has faced numerous headwinds throughout 2026, most recently revised downward guidance due to the slowdown of WATCHMAN products. While the stock is down -47.65% YTD, most of those losses came from cyclical pressures from hospital capacity and reimbursement dynamics rather than structural weaknesses. Management confirmed softer-than expected growth, slashing their full-year organic revenue growth expectations from an aggressive 10.5%–11.5% down to 6.5%–8.0%. The market heavily punished the stock, where it saw a 14% drop following shortly after guidance was revised down. BSX is still pending the $14.5 billion acquisition of Penumbra which is expected to close H2 2026, introducing further uncertainty revolving around the company's short-term financial strength. Despite those, the core structure and competitive position of the company remain intact, with Neuromodulation delivering 15% organic growth and Asia Pacific delivering 12% operational growth.

Valuation

Despite headwinds, the core profitability of BSX remained strong. Non-GAAP adjusted EPS rose 6% to $0.80, and organic growth rate increased by 9.4% (11.6% on a reported basis). BSX operates at 1.87x leverage, elevated from recent acquisitions, but expected to decline steadily as time goes on. Maintaining a large asset base of $44.35 billion expected to be bolstered by the $14.5 billion Penumbra acquisition, this asset base represents aggressive capital allocation that is geared toward M&A to bolster the competitive position of the firm. BSX currently trades at a 21.11x P/E ratio, compared to a five year average of 59.29x and a forward P/E of 14.62x. EV/EBITDA sits at 15.14x compared to a five year average of 29.92x, showing the stock looks very discounted on purely a multiple basis alone. Despite significant cyclical headwinds, adjusted EPS rose to $0.80 per share, a 6% increase compared to the $0.75 per share they reported in the same quarter last year, beating Wall Street consensus by $0.01. Revenue growth saw a 17.44% increase YOY as of Mar 31 2026, and revenue from WATCHMAN is expected to remain steady. Gross margins remain steady at 69.17%, and operating margins remain strong at 18.40%, higher than FY24 and FY23 numbers of 15.54% and 16.45% respectively. Ending 2025 with net debt/EBITDA near 2.3x, BSX maintains lower leverage than typical industry peers. The market has priced the stock as if it has seen significant structural impairment within the core functionality and profitability of the company, something guidance and the internal financial strength of the company do not support.

Business Quality

A wide range of acquisitions put BSX at leadership in numerous niche markets, allowing the company to capture market share in smaller but profitable sectors of healthcare. On May 18th, the firm invested $1.5 billion for an approximate 34% equity stake in MiRus LLC, gaining an exclusive option to acquire the business for an additional $3 billion, which is expected to close H2 2026. Recent clinical data revolving around WATCHMAN and Farapulse hope to expand the total addressable market for those products. BSX is leveraging its cash position to execute a $2 billion accelerated share repurchase (ASR) under its $5 billion total buyback authorization. BSX has seen increasing returns on its deployed capital, seeing 10.93% ROE and 9.42% ROIC as of Mar 31 2026, both above recent historical averages.

Sell Triggers Continued downward guidance on WATCHMAN revenues after cyclical pressures cede, coupled with two consecutive quarters of organic growth declines will signal the pressures that BSX faces may be more structural than currently expected. Fair value is assigned at $73, a mix of Morningstar fair value estimates and analyst consensus, with forward guidance revisions taken into account.
